Can't get this to work in playback. The quarter tones are being played as naturals.

of course, quarter tones are not being played, there is actually no specific sample for those notes, so you can add a midi message to move the pitch-bend wheel, first check if your patch (.nki or what ever )is programmed to follow the pitch-bend, use your controller for that, if it does changes the pitch, then you only have to automate the move..
to do so you add this midi message above the note: (use Ctrl+T)
~B56, 64 (center, no pitch change)
~B56, 78 (would be about quarter tone up)
~B56, 52 (would be about quarter tone down)
you have to set 64 again to reset for all notes, I know it is a mess, if you want the really extended method, you can edit samples one by one and create your own .nki for kontakt.
(message tested on Sib.7)

Okay, and that should effect just that specfic voice right?
I've not done anything with midi controllers in Sibelius yet, I'll have to figure out the interface.

No, pitch bend moves the entire patch and not just that, also be sure there's no other instrument in the same channel or will be affected too.
Is like keyboards, you move the pitch bend wheel and the entire keyboard changes not just the key you press.
First test if the message ~B56 works for you.

If you use Kontakt you can load two instances of the same instrument and lower or increase the tune of one of them by 50 cents. You should also add a second instrument in Sibelius which will only play the quarter tones and hide it. This is how I did it with my latest piece.
